Rate of Pay: $16.50/hour

Schedule: 40 hour/week

Location: Clitheroe Center

SCOPE OF POSITION

The Residential Case Manager provides direct case management services. Services generally include assisting clients to find housing and employment upon discharge, helping clients to make essential appointments, and working with the client to obtain other necessary resources.

QUALIFICATIONS



  1. Bachelor's degree in field related to social services preferred; one years' experience in the field of substance abuse, homelessness, or related field required in addition to a bachelor's degree. Five years' experience can be substituted for a bachelor's degree.
  2. Skilled in case management, discharge planning, housing and job search activities, and working with persons with mental health and substance use disorders.
  3. Knowledge of cross-cultural issues and DBH requirements.
  4. Understanding of trauma-informed care.
  5. Alaska driver's license with acceptable criminal history and driving records.
  6. Ability to maintain a collaborative and cooperative work environment and to work with a minimum of supervision.
  7. Proficient with general office equipment including computers and related software, with Windows applications, databases, and report-generating software.
  8. Familiarity with AK AIMS preferred.
  9. Ability to complete multiple tasks, meet deadlines, observe confidentiality, remain self motivated, and work with a minimum of supervision.



ESSENTIAL FUNCTIONS

The essential functions of this position include, but are not limited to the duties, responsibilities and physical demands below.



  1. Reaches out to residential clients to engage them in case management.
  2. Networks with other agencies and organizations which serve the target population to ensure availability of services for clients post-discharge.
  3. Works with landlords and housing managers to facilitate client placement in housing.
  4. Works with potential employers to obtain job leads for clients.
  5. Helps clients with housing and employment search.
  6. Assists clients in making necessary appointments for medical, dental, and legal services and for housing and job interviews.
  7. Assists clients to obtain needed prescription medications, clothing, and hygiene products.
  8. Assists clients as needed to apply for entitlement benefits.
  9. Reviews client data reports; attends case reviews and staff meetings regularly; uses evidence based practices and client feedback to improve services and treatment outcomes.
  10. Exercises assertive interaction skills to reach difficult clients; exercises trauma-informed care skills.
  11. Participates in training for the clinical staff.
  12. Coordinates services for clients on caseload and enhances intra-agency communication and cooperation.
  13. Conducts client interventions as appropriate within the scope of services of a Case Manager.
  14. Handles client holiday celebrations, including the client Christmas party and participation by clients in the Christmas GIFT program for clients' children.
  15. Participates in inter-agency committees and task forces as assigned by the supervisor.
  16. Attends regular supervision meetings with the Residential Clinical Manager.
  17. Documents client services in the electronic health records system.
  18. Performs other related duties as assigned.



WORK ENVIRONMENT

The majority of work is performed indoors. Noise level is usually at a quiet-to-moderate level.

PHYSICAL DEMANDS



  1. Talking, sitting, standing, walking, climbing, pushing, pulling, stooping, kneeling, bending, reaching on an intermittent or continuous basis
  2. Hearing and vision must be correctable to within normal ranges
  3. Ability to operate general office equipment.
  4. Ability to drive a motor vehicle in darkness or in inclement weather conditions
  5. Ability to lift up to 20 pounds without assistance

Education
Bachelors of Social Work (preferred)
Experience
1 years: In the field of substance abuse, homelessness, or related field required in addition to a bachelor's degree.
