HHA - Home Health (PRN)

Empath Home Health, a member of Empath Health, is seeking a PRN Home Health Aide (HHA). Providing compassionate, full life care is an honor we take seriously at Empath Health. Join our team and make a positive impact in the community!

The PRN Home Health HHA:

• Performs initial and ongoing comprehensive assessments of the patient's needs, including Outcome and Assessment Information Set (OASIS) assessments at appropriate time points.
• Initiates the plan of care and revise as necessary.
• Provides nursing services and/or treatments indicated on plan of care.
• Counsels and educates the patient and family on disease process, self-care techniques and prevention strategies.

Requirements:

• High school diploma or equivalent.
• Certification as a Home Health Aide.
• Minimum of one (1) year experience in home health care preferred.
• Current CPR certification.
• Valid driver's license and proof of automobile liability coverage.
• Able to communicate in English.
