General Purpose of the Job

The Project Coordinator, Clinical Programs (P1) supports catheter reduction and access progression initiatives by providing operational oversight of documentation integrity, workflow execution, and patient pathway progression across assigned processes.

This role is responsible for monitoring data accuracy, identifying gaps in workflow adherence, and supporting timely progression of patients through education, access planning, and intervention pathways. The Project Coordinator conducts ongoing surveillance of documentation quality and workflow performance, reinforces standard processes through targeted retraining, and escalates risks related to patient outcomes, compliance, or pathway integrity.

In addition, this role supports efforts to mitigate patient refusal, promote appropriate pathway designation, and ensure timely intervention for high-risk populations, including AKI patients. Through data-driven monitoring, teammate support, and structured escalation, the Project Coordinator promotes consistent execution, documentation reliability, and alignment with clinical and operational standards across the field.

Essential Duties & Responsibilities

• Manage data entry process and workflow tracking (30%)
  Oversee the data entry process for patient interactions, outreach attempts, and progression status to ensure information is captured accurately in designated systems. Maintain up-to-date workflow tracking to ensure clear visibility of patient status, support reporting needs, and enable effective coordination across teams.
• Support CWOW documentation integrity and surveillance (15%)
  Monitor documentation quality and workflow adherence to ensure accurate representation of patient interactions and progression. Conduct active surveillance using reporting tools (e.g., Tableau/Atlas) to identify gaps in required documentation, including education milestones, survey completion, and pathway progression. Provide targeted retraining to teammates on proper workflow execution and reinforce the importance of accurate documentation as a trigger for downstream processes. Escalate recurring documentation gaps to appropriate leadership when patterns of non-compliance are identified.
• Coordinate scheduling and outreach workflows (15%)
  Coordinate scheduling-related workflows, rescheduling processes, and reminder outreach using established procedures. Monitor workflow completion, identify gaps or delays, and ensure appropriate information is captured to support education completion and continuity of care.
• Support patient refusal volume mitigation efforts (10%)
  Monitor available reporting to identify trends and outliers related to patient refusal rates. Support targeted retraining efforts to reinforce patient engagement strategies and approved communication approaches. Partner with facility leadership to review patient-level opportunities and support development of action plans aimed at improving patient engagement and reducing refusal rates.
• Support permanent CVC pathway management and designation integrity (10%)
  Monitor patient-level workflow data to identify trends in pathway designations, including potential overutilization of non-candidate or permanent CVC classifications. Escalate concerns to appropriate clinical or operational leadership to ensure alignment with clinical expectations. Reinforce standard criteria and workflow requirements with field teams to promote appropriate pathway progression and documentation consistency.
• Support AKI intervention workflow adherence (10%)
  Monitor workflow activity and available reporting to identify delays in education, access planning, or pathway progression for AKI patients. Provide reinforcement of established protocols to ensure timely education and progression activities are initiated as expected. Escalate gaps in adherence to appropriate leadership to support timely intervention and alignment with standardized processes.
• Identify and escalate barriers to education and access progression (5%)
  Monitor workflow activity to identify gaps in engagement, delays in access placement, or incomplete education. Escalate barriers according to defined protocols and provide supporting documentation to enable timely intervention by appropriate partners.
• Execute teammate engagement and follow-up activities (3%)
  Complete assigned outreach and follow-up activities to support teammate participation in education and access planning. Adhere to defined timelines and scripts to ensure consistency and quality of engagement.
• Deliver structured training to teammates (1%)
  Provide standardized virtual or telephonic education using approved materials and scripts. Reinforce key concepts related to documentation, workflow execution, and access planning within defined scope and guidelines.
• Support team and operational workflows (1%)
  Provide coverage and coordination support as needed to maintain continuity of workflow execution across regions and teams.

Education and Requirements

• Typically requires 2-4 years of relevant experience in healthcare support, patient coordination, or administrative/operational roles
• Associate's degree required or equivalent combination of education and experience
• Bachelor's degree preferred

Certificates, Licenses, and Registrations

• No specific licensure required
• Relevant certifications in care coordination, patient engagement, or healthcare operations preferred

Travel and Time Requirements

• Travel: Up to 10%
• Nights/Weekends: Minimal; may be required based on business needs

Additional Requirements

Language Skills: Fluent in the written and verbal skills necessary to effectively educate patients and collaborate with cross-functional teams.
