Job Summary An essential, non-clinical member of the care team that provides services for patients with substance use disorder (SUD). The overall goal is to engage with patients, utilize harm reduction techniques to address any stage of readiness, provide resources, and help them address barriers as they work towards their goalsDoes this position require Patient Care? Yes Essential Functions -Consult and coordinate with healthcare team members to assess, plan, implement, and address barriers to patient care plans. -Facilitate connections across systems of care, including the recovery community, community-based supports, the addiction treatment system, and the healthcare system. -Engage with new patients to identify their needs and personal wellness goals. -Be available to regularly engage with patients outside of scheduled medical appointments, including introducing them to recovery-oriented systems of care or meeting them in their community. -Develop relationships with SUD-focused community groups/agencies and help patients access and navigate these resources. -Attends and participates in care team meetings. -Write brief, non-clinical notes in the patient's medical record.
