Overview

The Outpatient Care Management Supervisor assists the Outpatient Care Management Manager and Director by providing nursing leadership and oversight of the overall patient care process. This position supports the planning, coordination, and implementation of care delivery activities to ensure consistency, quality, and compliance with professional nursing standards. The role includes facilitating and coordinating patient care services, providing guidance and support to staff, and addressing operational or clinical needs as they arise. The Nursing Supervisor responds supportively to the needs of both staff and patients, promotes effective communication and collaboration, and assists with patient care duties as assigned.
Qualifications

Certifications: National Nursing Certification within 3 years - Varies; Licensed Registered Professional Nurse (RN) - Illinois Department of Financial and Professional Regulation (IDFPR); Basic Life Support (BLS) within 30 days - American Heart Association (AHA), Education: Bachelor's Degree: Nursing (Required), Work Experience: Registered Nurse
Responsibilities

Supports staff in resolving workrelated issues and addressing operational needs.
Assumes responsibility for personnelrelated functions, including interviewing, scheduling, payroll reporting, and enforcement of applicable policies, rules, and regulations.
Monitors employee performance and ensures timely communication of expectations and goals related to performance evaluations.
Ensures accurate clinical, financial, and operational documentation through established audit processes and appropriate systems.
Utilizes data, dashboards, and reporting tools to support high reliability, productivity, and evaluation of outcomes.
Promptly communicates significant patient care or personnel issues to the Nurse Manager and Director.
Assists leadership with continuous quality improvement, performance improvement, and outcomebased initiatives to ensure best practices across care management services within the Carle Health System.
Coordinates on boarding, orientation, and system setup for new employees.
Conducts monthly rounding with staff and supports dyad partner in rounding activities as needed.
Provides direct patient care as needed, comprising approximately 20% of job responsibilities, with the remaining 80% focused on supervisory and leadership duties. Maintains a collaborative dyad partnership with regional RN Care Management Supervisor. Demonstrates ongoing knowledge of Care Management best practices, CMS guidelines, and NCQA standards, and serves as the Epic Super User for the Care Management team. Collaborates with dyad partner to manage daily nursing operations in coordination with the OPCM Nurse Manager.

Compensation and Benefits

The compensation range for this position is $41per hour - $70.52per hour. This represents a good faith minimum and maximum range for the role at the time of posting by Carle Health. The actual compensation offered a candidate will be dependent on a variety of factors including, but not limited to, the candidate's experience, qualifications, location, training, licenses, shifts worked and compensation model.
