Under the direction of the Manager, Diabetes Education Program, the Diabetes Educator will be responsible for the assessment, planning, implementation and evaluation of Diabetes Self- Management Education and Support plans (DSMES) for patients and families with diabetes. Collaborates with PCPs and/or diabetes clinicians to develop and implement diabetes self-management education plans for patients with diabetes.

Major Responsibilities:

1. Utilizes patient education principles to assess, plan, implement and evaluate comprehensive DSMES plans for patients with diabetes.
2. Conducts an assessment of the patient's knowledge base of diabetes and readiness and motivation to learn. Identify any barriers to learning including age, physical. cognitive, environmental, cultural, language, financial, psychological and spiritual needs.
3. Based upon assessment, design an individualized DSMES plan based on the patient and multi-disciplinary teams mutually agreed upon goals.
4. Promotes continuity of care by working collaboratively with all members of the multi-disciplinary team in helping patient achieve diabetes goals.
5. Utilizes adult teaching-learning methods and a variety of presentation styles to provide education to patients and families.
6. Provides education based upon the clinical practice guidelines associated with the AADE 7 self-care behavior framework.: diabetes pathophysiology and treatment options; healthy eating; physical activity; medication usage; monitoring and using patient-generated health data; preventing, detecting and treating acute and chronic complications; healthy coping with psychosocial concerns; and problem solving
7. Provides advanced education regarding diabetes treatment modalities (insulin pumps, CGM) within scope of practice and state licensure.
8. Evaluates patient's response to treatment modalities and reassesses/revises plan of care as needed to help patient achieve goals.
9. Provides education through individualized sessions and group classes for 80% of scheduled working hours which may include Saturdays and/or evening hours.
10. Assists with keeping educational content and handouts current for both individual and class sessions. Registered Dietician educator maintains primary responsibility for medical nutrition therapy component.
11. Provides MNT when ordered: a thorough nutrition assessment including labs, medical history, nutrition related history, medications; formulation of nutrition related diagnosis; development of appropriate individualized interventions with evaluation/monitoring of progress made. The RD educator can prescribe a specific meal plan regarding diabetes and any other co-morbidities the patient has (i.e. renal disease, celiac, et).
12. Documents in a timely manner educational assessment, planning, intervention, behavioral goal setting and outcomes accurately in EMR in accordance with American Diabetes Association (ADA) recognition requirements. Demonstrates proficiency and compliance with UMMHC compliance and regulatory standards, insurance payer mandates and DCOE standards. Enters charges in a timely manner.
13. Educates healthcare providers and the community regarding diabetes through talks, health fairs, and being a resource regarding diabetes education for fellows, residents and students.
14. Demonstrates a commitment to provide quality education based on ADA recognition requirements.
15. Demonstrates professional commitment to diabetes education through membership in specialty organizations and maintaining/acquiring national CDE certification.

Position Qualifications:

License/Certification/Education:

Required:
1. Bachelor Degree from Accreditation Council for Education in Nutrition and Dietetics (ACEND) approved four-year program in Food and Nutrition or related field.
2. Certified Diabetes Educator (CDE) credential required within two years of hire.
3. Maintains a license with the Board of Dietitians and Nutritionists in the Commonwealth of Massachusetts.
4. Maintains registration with the Commission on Dietetic Registration.

Preferred:
1. Master's degree.

Experience/Skills:

Required:
1. Minimum of two years diabetes related experience.
2. Ability to influence the quality of patient care in an advisory capacity to the physician and other health care team members.
3. Demonstrated skill in analyzing problems and making decisions.
4. Oral and written communication skills are essential to interact with patients, staff, and referring providers.

Preferred:
1. Diabetes Education Experience in outpatient setting.
2. Experience with EMR.
