What You Will Do to Change Lives

Under direct supervision, the Case Manager II serves as a primary support and case manager for individuals with mental illness and other co-occurring disorders by assisting them to live as independently as possible in their chosen community.

Shifts Available:

Full Time; AM 8:30 am - 5:00 pm; Monday - Friday

Wage range $17.30 - $24.41 - Telecare applies geographic differentials to its pay ranges. The pay range assigned to this role will be based on the geographic location from which the role is performed. Starting pay is commensurate with relevant experience above the minimum requirements.

What You Bring to the Table (Must Have)

* High School Graduation or G.E.D. equivalent and two (2) years of direct services in Mental Health or community services; or Associates degree and one (1) year of experience; or Bachelor's degree in Social Services

* Valid and current driver's license, and personal vehicle insurance with your name listed as a driver.

* Willingness to use your personal vehicle to drive clients to appointments and groups, etc.

* Willingness to use the company vehicle to drive clients to appointments and groups, etc.

* Willingness to use your personal vehicle to attend meetings, etc.

VOICE: The California AB (Assembly Bill) 109 VOICE (Ventura Opportunities for Integrating into the Community Environment) program is an Assertive Community Treatment (ACT) model program that provides services to persons who are coming out of the California Department of Corrections (CDCR), are in need of mental health services, and have been designated to be supervised by the Ventura County Probation Department, rather than by parole. The services provided are outpatient and often take place in the field rather than in the office.

VISTA: Adult Forensic ACT (Assertive Community Treatment) program provides services to people that have been identified as severely and persistently mentally ill, homeless or at risk for homelessness, and incarcerated within the past year. Upon release from jail, a Telecare VISTA team member will be there to pick up the potential member, address immediate needs, and schedule an appointment for psychiatric assessment.
