The case manager is an integral part of the medical team. Follows the hospital stay and plans for the best discharge transition. Floor based assigment, teamed with an MD to provide high level assessment and planning for continued stays and discharges.

The Registered Nurse manages all aspects of the nursing process and the delivery of patient and family centered care. The Registered Nurse has the responsibility and authority to request and use governing and administrative resources, technological and support systems, and multidisciplinary clinical experts necessary to manage and deliver quality, cost effective patient care.

Skills: (Specific learned activity gained through training, e.g., typing, presentation skills, computer skills; e.g. Excel; CPR, ACLS)

* Required: Maintains clinical competency and current knowledge of regulatory and payer requirements to perform job responsibilities. Working knowledge of medical necessity criteria, such as Interqual. High degree of skill in written / oral communications and personal interactions. Demonstrates agility in a continuously changing fast paced work environment. Demonstrates computer competency. Knowledge of discharge planning process.

Experience:

* Required: 2+ years of appropriate clinical experience in utilization review or discharge planning experience preferred.

* Preferred: 4+ years hospital level care and discharge planning.

Education/Degree requirements:

* Required: Graduate of an accredited school in Nursing

* Preferred: Master's degree in nursing or social work.

Licensure, Certifications, or Registration:

* Required: Licensed as an RN in the state of Massachusetts.

* Required: BLS
