Summary:
The Cardiovascular Program Coordinator is responsible for coordinating and implementing services and activities related to the stroke and cardiovascular programs. The Coordinator actively cultivates collaboration, enthusiasm and mutual respect while planning, developing, implementing, coordinating, directing and evaluating the cardiovascular programs (stroke and chest pain) which are designed to optimize outcomes in care, maximize the consumer's involvement in his/her plan of care, improve service levels, reduce length of hospitalization and decrease costs. The Coordinator assumes joint responsibility with the nursing service, service line and program medical directors for achievement of program standards and ongoing quality management. The Coordinator is pivotal in integrating cardiovascular services throughout the healthcare continuum (pre-hospital, emergency department, inpatient and back into the community).

Responsibilities:


  • Demonstrates knowledge of program standards to obtain and maintain accreditation. Manages and facilitates designation requirements, associated documentation for accrediting and regulatory bodies and actual survey / assessment process. Collaborates with Quality Improvement on continuous improvement opportunities based upon data results. Serves as program data systems administrator.

  • Oversees comprehensive data collection, assuring data is complete and up to date and entered into the appropriate databases. Maintains current data entry for all cardiovascular registries including Action, Chest Pain and Cath/PC I and others as they become active. Performs data analysis and statistical reporting for programs. Collaborates with other health care providers to manage outcomes for assigned healthcare consumer population. Coordinates and oversees CP/Stroke data collection systems; assures that it is complete and up-to-date.

  • Coordinates and chairs (interprofessional) program care committees, including planning agendas, minutes, preparing data, etc. Develops educational programs for the healthcare consumer, family members and the community. Participates in community events pertaining to cardiovascular services for public awareness and educational purposes. Coordinates, in collaboration with Organizational Development team, educational and professional development activities to meet the learning needs of staff and EMS. Oversees the annual stroke and chest pain education requirements and ensures staff compliance / competency. Coordinates appropriate related education for staff (ACLS, BLS, etc.) and collaborates with Medical Affairs to facilitate and coordinate Medical Staff education.

  • Functions as a liaison for providers and teams caring for cardiovascular service patients. Coordinates and drives development of standardized plans of care, order sets, protocols, etc., based on evidence-based practices. Reviews clinical documentation to assure that clinical management recommendations and requirements are met. Monitors compliance with accreditation and regulatory requirements and reports data to service line leadership. Participates in discharge process to ensure national guidelines are being met. Assures development and follow-up of action plans to correct program deficiencies.

  • Collaborates with the Marketing department to promote services within the community. Participates in community events by serving as a program representative for community events and relevant committees. Establishes and maintains effective working relationships with community resources and service agencies.

  • Must be licensed to practice as a Registered Nurse in the state of North Carolina or one of compact states.

  • Bachelor's of Science in Nursing (BSN) or an equivalent combination of education and experience.

  • Three (3) years relevant ST Elevation Myocardial Infarction (STEMI) or Cardiovascular coordinator experience serving the needs of consumers with cardiovascular disease across the care continuum.

  • Direct clinical patient care experience in caring for consumers with cardiovascular disease.

  • Basic Life Support (BLS) certification

  • Advanced Cardiac Life Support (ACLS) certificationPreferred


Nice to Have


  • Master's of Science in Nursing (MSN)

  • Experience managing databases

  • Experience working in a healthcare quality role
