CLINICAL CASE MANAGER To provide services to adults with serious mental illness or co-occurring disorders This position works closely with the nurses, psychiatric providers, MH outpatient clinicians and our SUD team within the agency as well as community providers. We utilize a team approach and are expected to coordinate within the agency, community partners, and the family to meet the needs of the consumers in supporting independent living. The typical case load will be 40-50 cases. The qualified candidate must have good oral and written communication skills and strong interpersonal skills. Qualified applicants must have a high comfort level with technology. GPCS uses Credible, a web-based electronic health record. Current valid driver’s license and good driving record required. Bachelor’s degree required. A Bachelor’s or Master’s in a Human Service field is preferred. The candidate must meet the qualifications for Qualified Mental Health Case Manager. One to two years related experience in case management and working with individuals diagnosed with severe mental health illness, preferably in a Community Service Board or Behavioral Health Authority. Salary is commensurate with experience and qualifications, starting at $50,000/year.
