This position requires some field-based travel (up to 15%) in the Twin Cities area. You will be able to work remotely when not in the field.

The hours for this position are Monday through Friday, 8:00am - 5:00pm CST.

Primary Responsibilities:



  • Provide telephonic care coordination to MN Medicaid members with a mental health disability
  • Complete annual face-to-face meeting for qualified members
  • Manage gaps in care for members in both their mental health and medical needs (i.e., readmissions to higher levels of care, annual exams/services, etc.)
  • Manage member transitions to and from acute care settings (admissions/discharges)
  • Complete annual Health Risk Assessments and Care Plans for members
  • Coordinate and collaborate care with providers, facilities, and supports
  • Make clinically appropriate referrals for members
  • Coordinate and authorize medically appropriate services for members
  • Answer calls on the Special Needs Basic Care (SNBC) member queue and on the crisis line
  • Schedule and facilitate care conferences with member's providers
  • Work with internal clinical teams manage members' needs

Required Qualifications:



  • Master's or Doctoral Degree in Psychology, Social Work, Counseling, or related field; or Bachelor's Degree or greater in Nursing
  • Active, unrestricted, independent license in Minnesota: LP, LPCC, LICSW, LMFT or RN
  • 2+ years of clinical experience in a behavioral health setting
  • Live in or near the Twin Cities, MN
  • Ability to travel within the metro for annual member visits (up to 15%)
  • Access to install secure high-speed internet and a dedicated, distraction-free workspace at home



Preferred Qualifications:



  • Case Management experience
  • Experience working with Adults with Severe and Persistent Mental Illness (SPMI)
  • Experience with Substance Use Disorders (SUD)
  • Experience working in a strength-based recovery model
  • Experience with motivational interviewing
  • Proven solid organizational skills
  • Proven ability to work independently and manage a case load

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The salary for this role will range from $58,800 to $105,000 annually based on full-time employment. We comply with all minimum wage laws as applicable.
