Job Title: Social Worker, Inpatient Palliative Care Specific Duties And Responsibilities The Palliative Care Social Worker is a specialist level clinician who is responsible for social work practice on the palliative care consultation service. The successful candidate will demonstrate the ability to work collaboratively within the inter-professional palliative care team as well as with the primary patient care team. Essential responsibilities will include: Consultation with care primary teams for assistance with facilitation of family meetings, clarification of goals of care and plans of care in the setting of complex family dynamics and/or high emotional distress. Provision of institution-wide education efforts involving social work staff and social work interns to develop front line competencies in primary palliative care such as psychosocial assessment, interventions for symptom management, CBT, mindfulness, narrative therapy, transition planning and grief work. Participation in palliative care team coordination, triage and workflow. Participation in development and dissemination of protocols and training curricula to support the integration of palliative care throughout the medical center. Working closely with hospital leadership to enhance palliative care outreach to the community. Professional Licensures/Certifications: LCSW-C Preferred, Maryland Board of Social Work Examiners Supervisor Certification Preferred, ACHP-SW Preferred Why Join Us? Ready to make a difference? Apply today! Salary Range: Minimum $34.11/hour - Maximum $56.34/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility.
