Job Overview

Utilizing the nursing process, the registered nurse delivers comprehensive home health care to clients with various medical and surgical diagnoses in their place of residence.

Hours: Full Time/ Day Shift- weekend Rotation Required

Location: Lowell MA

Job Description

Minimum Qualifications:

1. Massachusetts RN Licensure

2. One (1) year of acute medical/surgical nursing experience

3. Valid driver's license, auto insurance

4.Current Basic Life Support (BLS) Certification.

Duties & Responsibilities: The duties and responsibilities listed below are intended to describe the general nature of work and are not intended to be an all-inclusive list. Other duties and responsibilities may be assigned.

  • Admits patients to the VNA who meet criteria for home health services.
  • Conducts initial assessments, including OASIS, to understand patient's health status and care requirements.
  • Collaborates with the patient, family, and other healthcare professionals to develop an individualized plan of care.
  • Communicates with physicians, other healthcare professionals, and caregivers to ensure a seamless and coordinated approach to patient care.
  • Maintains accurate and thorough and timely documentation of patient information, assessments, and care plans.
  • Complete all required admission documentation within agency-specified time frames.
  • Facilitates the transition of the patient and family to the Patient Care Team.
  • Conducts follow-up visits and re-evaluations to monitor the patient's progress and adjust the plan of care as needed.
  • Travels to and from patients' homes based on assignments.
  • Finalizes nursing admission assessments and visit plans within 24 hours of the initial visit.
  • Completes OASIS assessments within specified time frames for admission, reassessment, and discharge, making necessary edits.
  • Determines patient needs and provide clinical interventions based on assessments and medical orders.
  • Develops a comprehensive care plan and communicate it to the primary RN.
  • Evaluates patient progress toward goals and adjust or terminate services as necessary, ensuring proper notifications.
  • Understands principles of growth and development across the lifespan.
  • Utilizes communication skills to understand and respond to patient reactions.
  • Exhibits proficiency in care planning and treatment for various age-related illnesses.
  • Demonstrates awareness of the psychological and psychosocial needs of patients at different life stages.
  • Collaborates with patients, families, and healthcare providers on care planning and discharge.
  • Assesses physical, psychosocial, safety, and educational needs of patients and families.
  • Assists with end-of-life planning at home when appropriate.
  • Interacts with patients and families in a professional and compassionate manner while maintaining confidentiality.
  • Educates patients about next visit dates and their rights and responsibilities during admission.
  • Demonstrates awareness of safety and security concerns.
  • Provides education on home safety, including fire prevention, emergency action plans, and security.
  • Responds effectively to emergencies, following established protocols and utilizing stabilizing techniques.
  • Determines nursing interventions and procedures as part of the care plan.
  • Applies and educates patients on universal precautions.
  • Reports communicable diseases according to policy.
  • Educates patients and families on medication regimens, documenting progress.
  • Administers prescribed medications when indicated.
  • Ensures appropriate insurance authorizations for home care visits.
  • Adheres to VNA guidelines for releasing patient information.
  • Documents clinical findings, telephone contacts, and case conferences in patient records.
  • Monitors and evaluates patient progress toward achieving measurable goals.
  • Understands compliance concerns and report them to appropriate leadership.
