Requirements:

  • Required Certifications: State licensed, LSW, BLS, MSW
  • Required Skills: Develop plan of care to meet patient/caregiver needs, Development of problem list, Establish and revise goals, Review of Health history, Collaborative care, Coordinate services to home care and equipment following discharge, Coordinates quality care in cost effective manner, Coordinates with internal and external team members, Documents pertinent information to facilitate care coordination, Participates in care conference activities, Refers to rehab and long term care facilities, Refers to other disciplines/resources, Supervises staff effectively and efficiently, Discharge Summary, Documentation according to plan of care, Documentation of care delivery, plan, goals, etc, Documentation of patient/caregiver teaching/learning, Documentation reflects understanding of coverage criteria, Documents patient response, Facilitate discharge and contingency planning, Understanding and utilization of short-term therapy, Educates patient/caregiver to facilitate discharge, Provide / reinforce individualized patient/caregiver education, HMOs, Medicaid, Medicare, Medical Transportation, Medication Assistance, Prior Authorization, Agency on Aging, HCBS (Home and Community Based Services), Medicare waiver, Admissions, Arrange transfers to physician appointments, and discharge home, Complete all paperwork and accountability reports, Discharge Planning, Facilitate social and psychological well being of nursing home residents, Plan of Care, Advance Directives, Americans with Disabilities Act, High risk intervention patient vulnerability or safety risk, Knowledge of cultural issues, NASW Code of Ethics/Standards of Home Health Care, Patients with communication problems (hearing, speech), Admission Criteria, Appeals and Denials, Benefits Eligibility, Care coordination, Concurrent Review, Continued Stay Reviews, Discharge Planning, Needs Assessment/ Order DME, Plan of Care, Pre-Cert Review, Prior Authorizations, Worker's comp case management

Job Details:

  • Pay: $2175/week - $2430/week
  • Assignment Length: 8-week assignment
  • Shift: 5, 8-Hour 09:00 - 17:30
  • At least one year of experience required
