POSITION SUMMARY:

The Autism Transition Specialist will work in conjunction with the clinicians and staff in Developmental and Behavioral Pediatrics to support, provide information and offer ongoing consultations to families of adolescents and young adults diagnosed with Autism Spectrum Disorders (ASD). Responsible for outreach to families, meeting with families during clinic sessions, assisting families in interactions with school system and community appointments agencies and documentation of notes as per clinic guidelines.

ESSENTIAL RESPONSIBILITIES / DUTIES:


  • Provides information and guidance on how to obtain resources available through outside agencies, within BMC and written/web-based resources to adolescents/families of 14-22 years olds diagnosed with ASD.
  • Maintains positive relationships and ongoing communications with collaborating and consulting health care providers, agencies and school personnel in order to review services available and to obtain information.
  • Assists families in interacting with school personnel and agencies.
  • Assists in completing appropriate forms and documentation.
  • Provides ongoing input regarding child's educational progress and programming.
  • Works collaboratively with parents to develop culturally and linguistically appropriate supports based on family priorities.
  • Conducts resource trainings/outreach initiatives for professional and parent audiences.
  • Seeks community collaborations and partnerships to strengthen supports available to families.
  • Educates pediatric providers about existing resources and benefits for families Helps families to solve problems that interfere with obtaining care and support for their child and family.
  • Attends and participates in staff meetings, clinical sessions where appropriate and committee meetings.
  • Assists team in making referrals and supporting family with follow through. Co-coordinates TEAM program (Teens Engaged as Mentors).
  • Responsible for organizational oversight including but not limited to participant recruitment, collection of application and participation materials, planning and facilitation of monthly events, participant and family correspondence and ongoing assessment of program impact.
  • Demonstrates professionalism.
  • Communicates and documents appropriately including notes and records in the EMR and related databases.
  • Performs other related duties as assigned.

JOB REQUIREMENTS

REQUIRED EDUCATION AND EXPERIENCE:

Experience working with individuals with developmental disabilities and their families.

Master's degree preferred in psychology, mental health, special education, or applied behavior analysis.

  • Strong organizational, interpersonal, and computer skills.
  • Must know all Microsoft Office programs and have facility with web to access information.
  • Work requires communication skills and well-developed writing skills in order to effectively deal with conflicting views or issues and mediate fair solutions.
  • Work requires analytic skills to collect information from diverse sources, apply professional principles in performing various analyses, and summarize the information and data in order to solve problems.
