The Department of Geriatric Medicine seeks to hire an Advance Practice Provider (NP or PA) to join their Inpatient Consult team at Corewell Health West providing care to complex hospitalized patients in Grand Rapids, MI.

Provides consultation on the specific needs of older inpatients (currently trauma & orthopedic trauma patients), ensuring that the patient and family's goals of care are identified and honored as well as facilitating appropriate medication management and discharging planning.
