Overview

With general direction and regular consultation with the Executive Director the Manager of Community Health Systems assumes primary responsibility, supervision and leadership for the development, support and provision of the Community Care Program/Case Coordination Unit for the Vermilion County, and Senior Information Systems, Coordinated Point of Entry for both Vermilion and Champaign Counties. For the CCP, Manager oversees staff who perform professional assessments of the physical, social and economic needs of individual program applicants and/or clients and determines financial eligibility for these services, as well as the Choices for Care client screening for possible long-term placement. For Senior Information Services/Coordinated Point of Entry, Director oversees all aspects of service delivery, education, outreach, data collection, and reporting within the SIS department. The Manager will assist the Executive Director in supporting roles as assigned.

Responsibilities

Designs and conducts needs assessments to determine the nature and extent of a wide variety of human services, and care and delivery systems; checks applications for accuracy and completeness; provides Executive Director and Board of Directors information on staff including ensuring compliance with professional standards.
Conducts program evaluation on behalf of the agency; oversees, directs or designs forms for the agency staff to complete, and analyzes written reports from the staff and funders; develops and oversees site visit schedules for the staff and maintains clear communication with the Executive Director on findings.
Oversees the development/implementation of and review of services contracts from different funding sources; reviews contracts as submitted by agencies and manages requirements as stated in contracts for CCU/SIS and related programs.
Works with other agencies, nursing homes, senior housing facilities etc. including public and private agencies, hospitals, community groups, organizations, and other county departments to promote and direct an approach for CCU/SIS service planning and conflict resolution; attends and speaks at meeting and provides information to the CEO.
Develops and disseminates educational materials and tools for clients.
Builds and maintains effective relationships with project partners across the continuum of care.
Ensures compliance and provide training on HIPAA in CCU and SIS.
Reviews and submits monthly billing statements to CRIS Healthy-Aging Center.
Provides regular progress reports and outcome data to project partners.
Meets initial and ongoing training requirements and ensures coaches maintain the required training.
Hires, manage, and oversees CCU/SIS staff.
Oversees Community Outreach staff in the absence of the Director.
Implements and oversees CCU case management services as designated in the Illinois Department on Aging Case Management and East Central Illinois Area Agency on Aging manual for person's age 60 and older.
Updates CCU case management staff when there are changes with IDOA and ECIAAA. Assures the CCU is in compliance with the rules and regulations of IDOA and ECIAAA.
Assists and oversees the monthly billings, quarterly report development and completion of the contract planning.
Oversees and provides appropriate type of onsite/in home assessments to determine needs eligibility.
Oversees staff with development of care plans and arranges in-home services, adult day services and/or other appropriate services to maintain appropriate support for the client.
Works with staff to make appropriate referrals to other agencies when necessary.
Maintains a working knowledge of the state/regional rules and regulations.
Assures staff completes required paper work within timelines designated by IDOA, ECIAAA, Partner Hospitals, and CRIS.
Establishes and conducts CCU/SIS meetings and in services.
Oversees the SIS department in collaboration with the SIS Supervisor/ Communications Coordinator
Monitors staff to ensure clients are provided follow-up services in a timely manner.
Reports incidents of elder abuse to designated Elder Abuse and Neglect Agency.
Attends and ensures staff maintains the required training of IDOA and ECIAAA.
Completes Adult Protective Service Supervisors training.
Maintains knowledge of Senior Information Service (SIS), Caregiver Program, Senior Health Information Program (SHIP), Coordinated Point of Entry (CPoE), and Money Management.
Corresponds regularly with all CCU providers as well as IDOA to maintain compliance.
Maintains knowledge and expertise with Computer software applications and Word processing programs.

Compensation and Benefits

The compensation range for this position is $32per hour - $55.04per hour. This represents a good faith minimum and maximum range for the role at the time of posting by Carle Health. The actual compensation offered a candidate will be dependent on a variety of factors including, but not limited to, the candidate's experience, qualifications, location, training, licenses, shifts worked and compensation model.
