Suncoast Hospice of Hillsborough, a member of Empath Health is currently seeking a compassionate, caring and committed Admissions Registered Nurse (RN) to join our team covering Brandon area and Polk, Hardee and Highlands counties.

Shift: Thursday-Sunday 10am-9pm

The Admissions RN is a first responder who assesses eligibility for Empath Health Network of Care while advocating for optimal access to the right care at the right time in any care setting. The Admissions RN completes the admission process based on care program specifics, guidelines, and patient-specific needs. This RN exhibits passion for the services we offer while engaging in mission support, and portrays excellence in customer service and relationship-building internally and externally.

JOB DUTIES/RESPONSIBILITIES:

  1. Completes agency as well as departmental Nursing Competencies according to specifications.
  2. Performs the Initial Patient/Family contact to assess eligibility for any program.
  3. Assumes responsibility for accurate level of care and determination of financial classification for newly admitted patients.
  4. Completes admission process in accordance with care program specifics and agency guidelines.
  5. Completes documentation within electronic medical record in accordance with agency policies, practices and HIPAA.
  6. Communicates admission information in accordance with Communication Practices.
  7. Follows Empath Health's Communication Guidelines for inter-agency communication at all times.
  8. Assumes responsibility to perform assessment and admission process in all care settings.
  9. Assumes flexibility for travel within Empath territories or neighboring areas, depending on departmental needs and scheduling.
  10. Coordinate and assist with the Professional Liaison to deliver educational, in-service and outreach tools in the hospital.
  11. Partner with the Professional Liaison to deliver formal and informal education opportunities with physicians and hospital social workers/discharge planners/patient unit staff.
  12. Provides input on policies/procedures as related to admissions and/or nursing programs.
  13. Works directly with Admissions/Care Navigation Leadership to promote rapid and optimal access within our service area as well as meeting the needs of those we serve.
  14. Works directly with community referral sources to educate regarding Empath Health programs and services.
  15. Works directly with Empath Health physicians regarding patient eligibility, status, focused needs, care plan.
  16. Works directly with inter-agency departments to coordinate optimal care for all new admits while insuring a smooth transition from prospect to patient.
  17. Understands and adheres to customer service excellence, best practice and HIPAA guidelines continually.
  18. Complies with RN On Call scheduling needs as well as vendor credentialing process.
  19. Performs other duties as assigned by Admissions/Care Navigation Leadership.

POSITION QUALIFICATIONS/REQUIREMENTS:

  • Education and/or Experience: Associate's degree (A.A.) or equivalent from two-year college or technical school; Minimum of or six months to one year related experience and/or training; or equivalent combination of education and experience.
  • Licensure:
    • Valid Florida license as a Registered Nurse
    • Valid Florida State Driver's License
  • Certifications: Valid CPR certification from the American Red Cross or American Heart Association.
  • For field-based positions, employees must have reliable transportation which will enable them to perform tasks and responsibilities in a timely and appropriate fashion. Must provide proof of valid automobile insurance, a copy of which will be placed in the employee's HR file.
