Job Details Job Description Clinical Patient Navigator for the Cancer Program participates on the multi disciplinary team as an advocate, interpreter, educator and counselor for oncology patients (medical, surgical and radiation). Responsible for ensuring all patients receive quality and comprehensive services. Coordinates care throughout the continuum in collaboration with team. Serves to identify barriers and find resources to assist patients. Serves as a liaison throughout the network and in the community regarding available services. Assist patients in understanding their diagnosis, treatment options and available resources. Educate patients and their family Serves as liaison between patients and care providers Facilitate appointment and support services Coordinate collection of data and tracking of outcomes with cancer registry staff. Coordinates care conferences preparing concise patient summaries for use by care team.
