The Medication Safety Pharmacist position will be responsible for the following:* Develop and facilitate implementation of the annual Medication Error Reduction Plan (MERP) initiatives* Organize and coordinate the individual MERP subcommittees: Pediatric, Neonatal/OB, Periop/Procedural, Emergency Department, Medicine, Behavioral Health * Oversee and lead monthly Safe Medication Practices (SMP) sub-committee meetings* Coordinate and oversee follow-up actions from SMP sub-committee meetings* Oversee and facilitate medication related iReport resolutions* Assist in medication delivery pump library updates* Quarterly on-site medication safety audits at UCSD Health pharmacy sites Additional roles may include but are not limited to:* Advise and coach staff to identify resolutions for medication safety issues* Interpret, update, and manage pharmacy department and health system policies as related to medication safety* Directly observes and analyses the medication use process* Precept pharmacy students, PGY1 pharmacy residents, and PGY2 pharmacy residents The Medication Safety Pharmacist will spend 20% of their time in medication use/operational activities Cover letter is required when applying to this position.
