What You Need to Know

The Care Transitions Nurse is responsible for partnering with assigned facility(ies) to achieve optimal patient satisfaction & outcomes by facilitating successful patient transition from facility to home. The role is responsible for driving referral growth by educating key personnel on AccentCare programs in assigned facility(ies).Our Investment in You

Qualifications

Be the Best Care Transition Nurse You Can Be.

If you meet these qualifications, we want to meet you!

  • 1 - 3 years of experience in facility relationships and an understanding of facility discharge processes
  • Nursing degree preferred
  • Licensed RN or LVN in practicing state
  • Current driver's license and liability insurance
